Ensures that drugs and/or devices are dispensed and delivered safely and accurately. This is accomplished by identifying potential and actual drug-related problems, resolving actual drug-related problems, preventing potential drug-related problems, and optimizing patient therapy outcomes. Directs and coordinates pharmacy and site operations. I. ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned. • Ensures, as Pharmacist-in Charge, that the pharmacy meets all state and federal laws and regulations. • Promptly and accurately receives, interprets, evaluates, prepares, and dispenses prescription drug orders. • Provides to the patient or patient’s agent accurate and meaningful verbal and written drug and/or device information. • Complies with Healix Policies and procedures and recommends modifications as necessary. • Selects cost-effective and clinically appropriate drug products, methods of delivery, and delivery systems as permitted by prescribing physician. • Develops, implements, and/or enhances pharmacy care programs and/or services that enhance and add value to the client’s practice. • Maintains optimal drug and supply inventory and reports are submitted in a timely basis. completion of annual controlled substance inventory. • Insures that contracted drugs are ordered, received, stored, and secured properly. • Insures that adequate security measures are in place to prevent indiscriminate or unauthorized access to pharmacy and that confidential patient records are maintained. • Communicates effectively with client. • Participates in pharmacy staff meetings and Journal Club on a regular basis. • Submits all real-time sales, DME tracking forms, patient rosters, and inventory reports on a timely basis. • Demonstrates a “team player” attitude with co-workers (on-site and corporate). • Assures that all on-site personnel, including coverage personnel where appropriate, are current in all validation, training, etc., and are documented. • Tests and reviews the aseptic techniques of onsite staff to assure adherence to aseptic procedures. • Maintains clean and neat pharmacy. • Develops and maintains written protocols for dispensing and/or drug therapy management as needed. • Insures that patient charts are maintained, current, and complete according to state rules and regulations, JCAHO standards, and Healix policies and procedures. • Responds to pages while on-call within 15 minutes. • Maintains ongoing quality improvement within the site. • Maintains and renews all licenses held by the pharmacy in a timely manner, on behalf of the client. • Coordinates total patient care needs with the physician and nurse. QUALIFICATIONS • EDUCATION and/or EXPERIENCE o Bachelor of Science (B.S.) in Pharmacy or Doctor of Pharmacy (Pharm.D.) o Two to three years of related home care or hospital experience preferred. o Meets all requirements of Staff Pharmacist position. • CERTIFICATES, LICENSES, REGISTRATIONS o Assigned State Board Pharmacy License o ACPE Sterile Products Certification • OTHER SKILLS AND ABILITIES o To perform this job successfully, an individual must have experience in home health care/hospital pharmacy management. o The individual must also have experience with JCAHO.
