Complies with hospital/departmental policies and procedures, i.e., attendance, mandatory education, safety, infection control, etc. 2. Assists in coordinating and supervising departmental functions. • Takes responsibility to see that required daily functions are completed when working as “MAIN” pharmacist (drug purchases, drug charges, floorstock replenishment, etc.). • Supervises the pharmacy support staff in a consistent, fair manner and leads by a professional example. • Effectively communicates important issues with oncoming shift. • Resolves conflicts professionally. • Maintains departmental adherence to state and federal law and professional standards. 3. Accurately and effectively provides medication therapy. • Prioritizes work. • Appropriately reviews physician order(s) and inputs into the pharmacy profile system. • Reviews drug regimen for drug allergies, rational therapy, contradictions, reasonable dose and route, reasonable directions, duplication of therapy, drug-drug interactions, other interactions, and adverse drug reactions. • Applies knowledge of pharmacotherapy to identify patient and family requirements relative to age-specific needs (see age-specific criteria). • Communicates with other disciplines and the patient to resolve issues. • Appropriately check all medications prior to dispensing. 4. Actively participates in intervention, performance improvement and professional programs. • Makes or recommends appropriate therapeutic interchanges. • Reports Adverse Drug Reactions and Medication Errors. • Provides consults upon request. • Provides patient medication counseling upon request. • Provides drug information upon request. • Assists in training, educational, and quality assurance programs. • Serves on hospital teams, as directed. • Provides medication in-services, as directed. • Completes special projects as assigned by Director. • Offers constructive recommendations for improving departmental performance. 5. Employee’s conduct must reflect the Company’s values and a commitment to the Code of Conduct ethics and compliance program. 6. Other duties as assigned by the Department Director or Senior Pharmacist. Skills/Requirements EDUCATION AND EXPERIENCE: Required: - A Bachelor of Science in Pharmacy or a PharmD Basic Computer Skills Preferred: A Bachelor of Science in Pharmacy or a PharmD. - Previous & recent hospital experience. - Knowledge of CPCS, MediTech, AcuDose, Microsoft Office 1 year min hospital experience. LICENSES AND CERTIFICATES: Required: - Current licensure in Texas as a Registered Pharmacist. Preferred: Current licensure in Texas as a Registered Pharmacist. BCLS QUALIFICATIONS: Education: B.S. or higher degree in Pharmacy from an accredited university. Experience: Minimum 1 yr recent expereince as staff pharmacist in an institutional setting, with unit dose, IV and TPN background. Must have Neonatal experience or a Residency with a Neonatal focus or extra rotation in the NICU. Computer entry skills recommended. Demonstrates professional manner and possesses good communication skills.
